We collect your following personal information when you sign up or open an account or electronically submit to us your inquiries or request in relation to Mandaluyong Contact Tracing System:
Public and Private Establishments:
Your collected personal information is utilized solely for COVID-19 contact tracing, health updates, emergency response, as well as for vaccination program registration of the city in compliance with Ordinance No. 814, Series of 2021, and are not shared with any outside parties other than the authorized App developer Appcase, Inc. and other LGUs that we have signed interconnectivity and data sharing agreements for the purpose of contact tracing only. These information enable us to properly process your online registration, inquiries and request, forward to Mandaluyong City Epidemiology and Surveillance Unit (MCESU), Department of Health, Data Privacy Committee, or the Data Protection Officer, for appropriate action and response, and provide you with appropriate updates and advisories by personal service or interaction, e-mail or telephone call in an orderly and timely manner. In accordance with your communication preferences, we will occasionally contact you to provide update status of your registrations, announce new features we build for you, if may, inform you of mandatory COVID-19 protocols, and share tips and information.
The City Government of Mandaluyong does not share the registrants’ personal information with any third party for advertising purpose. Only authorized personnel of the City Government of Mandaluyong and its App developer Appcase, Inc., have access to personal information processed herein. The personal information collected by http://mandatrack.appcase.net are stored in our database and Cloud Storage until 12 September 2021 pursuant Presidential Proclamation No. 1021 unless the State of Public Health Emergency shall be further extended by the President of the Philippines, or unless otherwise provided under the law or ordinance.
Personal Information collected from registrants shall be delete in our data base and Cloud Storage immediately after lifting of State of Public Health Emergency by the President. However, information of COVID-19 positive registrants shall be archived or stored by the MCESU for reference and documentation purposes. Scanned information of QR Codes shall be transmitted and integrated to the Mandaluyong Central Contact Tracing System. It shall be deleted every thirty (30) days except scanned information of QR Codes of COVID-19 positive registrants which will be stored and archived by the MCESU for reference and documentation purposes only.
Personal information that are digitally processed by MandaTrack are encrypted using Advance Encryption Standard with a key size of 256 Bit (AES-256) and 2048 Bit (RSA-2048) to ensure data protection. MandaTrack is also using an Application Program Interface (API) imbedded and implemented on a separate site protected with an authentication and authorization method. The App developer already conducted Vulnerability Assessment and Penetration Testing on the MandaTrack. Confidentiality and nondisclosure agreements are likewise executed by the App developer, contact tracers and those individuals involved in the collecting and processing of personal information in connection with the MandaTrack.
Accessing and correcting your personal information
Should you wish to access your personal information found in the official records or database of the City Government for the MandaTrack for the purpose of correcting, completing, deleting or blocking any, some or all of your personal data. To do so, a written request in the prescribed form shall be addressed and filed with the City’s Data Protection Officer (DPO) or send such request at firstname.lastname@example.org.
Filing of complaints and inquiries
Pursuant to Ordinance No. 814, S-2021 and its implementing rules and regulations, all complaints and inquiries relating to the MandaTrack and data privacy violations in connection thereto shall be addressed and filed, in writing, with the Data Privacy Committee with office address at 4/F Executive Building, City Government Complex, Maysilo Circle, Plainview, Mandaluyong City.
